Section 1: Relationship History

Length of Relationship:10 years and 4 months
Reason for Separation:Irreconcilable differences. The parties grew apart over time and were unable to resolve ongoing disagreements about parenting approaches and household responsibilities.
Section 2: Current Arrangements
Where children currently live:The children currently live with me at 42 Sunset Drive, Scarborough WA 6019. They have their own bedrooms and attend Scarborough Primary School.
Current time with other parent:The children currently see their father every second weekend. Changeover occurs at McDonald's Scarborough at 5pm on Fridays and Sundays.
How is this arrangement working:The current arrangement is working reasonably well. The children enjoy seeing their father but sometimes express that they miss him during the week.
Section 3: Safety Concerns (if any)

Tips
  • Must be signed in front of a Justice of the Peace, lawyer, or court staff
  • Include specific facts with dates - not opinions
  • Attach relevant documents as exhibits (school reports, medical records)
